Show Form Completion Notification on Actual Completion Date (Not Original Schedule Date)

Right now, if a client completes a form late, the notification shows up under the originally scheduled date instead of the day it was actually completed.
Why this is a problem:
Coaches can easily miss late submissions because they get buried in past notifications.
This makes it harder to track client activity in real time, especially if there are lots of notifications coming in.
Requested improvement:
Have the notification appear on the day the form is completed.
Optionally, display the original scheduled date in brackets (e.g., “Completed Sept 25 (Originally scheduled Sept 21)”).
This would make notifications more accurate, reduce missed client submissions, and give coaches a clearer view of real client activity.
